Process Flow from Application to Hospital Visit
- After applying by email (borntobenephro@naver.com) for treatment, you will receive a provisional application form.
- Please insert required information to the Transient Dialysis Reservation Application and send it back E-mail.
Within a week you will receive a reply to inform you if you can be treated on your requested date.
Please send back your filled Transient Dialysis Reservation Application at least 3 weeks before your request date.
- If treatment is possible on your requested date, we will send you a Patient Information Sheet.
Please also insert all the required information, and enclose a copy of your latest 3 dialysis records.
Please send us the documents at least one week before your requested treatment date.
- Please visit to the hospital reception on the reserved day and pay the treatment fee.
After the procedures, you will be taken to the dialysis room.
Payment
At the Borntobe medical center we accept Korean WON in cash, or credit cards.
Accepted credit cards are
VISA, Mastercard, JCB, American Express
Cancellation Policy
If you wish to cancel your treatment please contact us ahead of time.
The treatment fee will be charged in case of absence without prior notification.
Please also be aware that if we do not receive the required documents 3 days prior to the treatment date, the reservation will be cancelled.
Others
You will be informed about the fees for Hemodialysis, Hemodiafiltration, or Erythropoetin (in case of administration) and are requested to pay upfront.
In case additional medical treatment is needed during the dialysis, the additional fee can be paid after the treatment
